Apparently, the various PBS stations can pick up some shows but don't have 
to carry all of them.  Way back when, one of the local PBS stations was
part of the statewide Kentucky Educational Television (KET) network while
the other was an independent PBS station.  IIRC, the programming available 
between the two varied wildly.  Now they are both part of KET.

That could have been.  Slight spoiler but what wound up happening was that 
Martin was convinced she didn't need it, and either he (or his Aunt Ruth,
also a doctor) convinced her to go without it.  She then had a serious
problem whereupon it was realized that she did need it!

To my recollection, no there was no such character.  I don't remember any
characters, aside from his first Aunt, who remembered him being in Port
Wenn as a child.
 
The cop, Mark Milo, is radically different.  He reminds me a little more
of Penhale (the second cop) but he is a lot different than him still.  I
don't think the couple that runs the Salty Breeze had any real parallel
characters in the original, either.  
 
I think they have changed it a lot for the American audience.  In the
original, Martin seems to have an issue (maybe slightly on the autism
spectrum) that makes him oblivious to social cues.  This version seens to
be capable of picking up cues but just doesn't like being social.
 
I also don't think we ever met Elaine's mother in the original.
 
Without spoiling it, the main story line in Episode 3 has a slight
parallel with a much later episode in the original series, but that is
about it.  Very little of what happens reminds me of the original.
